This form must be completed by the Headmaster/Headmistress of the student’s current school to evaluate key attributes such as punctuality, collaboration, respect, trustworthiness, and leadership.
Key Instructions:
- Circle responses for each assessment criterion.
- Provide details on leadership roles, suspensions, fee payment history, and a recommendation for admission.
- Seal the completed form in an envelope, sign across the seal, and either post it to the Headmaster (P.O. Box 9533, Dar es Salaam) or hand it securely to the applicant.
- Submit preferably before the end of the application period (check current deadlines on www.loyola.ac.tz).
This form ensures a holistic review of the applicant’s character in line with Jesuit values.
