As part of the entrance examination application process for Form II and Form III (2026 intake) at Loyola High School, a Confidential Character Assessment Form is required. This form must be completed by the Headmaster/Headmistress of the student's current school to evaluate key attributes such as punctuality, collaboration, respect, trustworthiness, and leadership.
Key Instructions:
- Circle responses for each assessment criterion.
- Provide details on leadership roles, suspensions, fee payment history, and a recommendation for admission.
- Seal the completed form in an envelope, sign across the seal, and either post it to the Headmaster (P.O. Box 9533, Dar es Salaam) or hand it securely to the applicant.
- Submit preferably before the end of the application period (check current deadlines on www.loyola.ac.tz).
This form ensures a holistic review of the applicant's character in line with Jesuit values.

Form III Entrance Examination Application Form – Download Here
Loyola High School invites applications for the Form III Entrance Examination (transfer/continuation entry for 2026).
Download and complete the application form below. Submit the filled form to the Registration Office (Administration Block, Room A009) along with all required documents by the announced deadline.
Requirements include:
- Photocopy of Birth Certificate or affidavit
- Photocopy of recent Academic Progress Report
- Two passport-size photos
- Confidential Character Assessment from current Headmaster/Headmistress
- Proof of payment (non-refundable TShs. 30,000/= form fee)
- Bank payment to CRDB A/C 0150315988101 (LOYOLA HIGH SCHOOL) or LIPA NAMBA (M-PESA) 5132307
- Attach bank pay-in slip or payment evidence
Forms downloaded from the website must include payment proof—without it, the application will not be considered.
All subjects at Loyola are compulsory for admitted students.
