Loyola High School invites applications for the Form III Entrance Examination (transfer/continuation entry for 2026).
Download and complete the application form below. Submit the filled form to the Registration Office (Administration Block, Room A009) along with all required documents by the announced deadline.
Requirements include:
- Photocopy of Birth Certificate or affidavit
- Photocopy of recent Academic Progress Report
- Two passport-size photos
- Confidential Character Assessment from current Headmaster/Headmistress
- Proof of payment (non-refundable TShs. 30,000/= form fee)
- Bank payment to CRDB A/C 0150315988101 (LOYOLA HIGH SCHOOL) or LIPA NAMBA (M-PESA) 5132307
- Attach bank pay-in slip or payment evidence
Forms downloaded from the website must include payment proof—without it, the application will not be considered.
All subjects at Loyola are compulsory for admitted students.
